REQUIRED NOMINATION DOCUMENTS
NOTE: Please pay close attention to the word limits on all documents. To ensure equal treatment, additional documentation or documentation over the specified word limits will not be considered.

This year, the nominator may EITHER submit a nomination letter documenting the nominee's fulfillment of the nomination criteria OR complete a Mach Form:

Note that only one of the Nomination Letter or Mach Form are required but all other nomination documents must be attached.

Your letter should highlight how the evidence in this nomination fulfills the nomination criteria (see Link) (maximum 750 words).

The statement should demonstrate that the nominee has fulfilled the required criteria (maximum 2000 words).

These evaluations could be regular in-course assessment instruments or information collected expressly for the nomination to be provided by the nominee (maximum 5 pages).

The CV should highlight the nominee's teaching experience and accomplishments. Contact the nominee's department or the nominee for this document (maximum 5 pages).

LETTERS OF SUPPORT
Three letters of support (maximum 500 words), two from current students or alumni in all areas of the nominee's teaching, and one from a colleague familiar with the individual's teaching activities who, in the case of TAs, can speak to the teaching, professional development/mentorship requirement.
